In addition to completing the form below, you will need to provide the following in person at the library or a photo by email:
- Photo ID.
- Proof of current address (examples: updated driver’s license, water bill, rental agreement, or mail you’ve received at your current residence).
- If you are are under the age of 14, you will need to have a parent create your card for you.
Anyone can get a library card and have access to all physical items in the library and local library services. Cardholders who live within West Branch city limits or unincorporated Cedar County have access to downloadable ebooks through Bridges.
